Location: Metlakatla, BC - Onsite

Reporting to: Health Director

POSITION SUMMARY

As the Community Health Representative (CHR), you will work closely with the Community Nurse to establish community health priorities, including prevention, detection, assessment and treatment. The CHR is responsible for delivering high standard Community health, treatment, and surveillance programs to provide quality health prevention and treatment in the community. Additionally, you will be responsible for liaising between patients, families, the community, and healthcare providers to ensure patients and families understand their conditions and treatment.

Responsibilities:

Support and Liaison

• Develop, deliver, and evaluate Home Care programming in partnership with Health Centre staff, including referrals, eligibility screening, care planning, and resource recommendations

• Act as a community resource and liaison to help clients and families access appropriate health services and understand care options

• Use culturally sensitive, strengths-based approaches to support health concerns, care coordination, and communication with caregivers and service providers

Implement Plan of Care

• Work closely with clients, families, physicians, and healthcare providers to develop and implement care plans that reflect client needs, goals, and values

• Prepare clear correspondence, charting, teaching materials, and reports to support communication with clients, colleagues, managers, and health professionals

• Collaborate with Health Centre staff and other professionals to support appropriate client care, continuity of service, and emergency preparedness

Evaluate Care

• Complete required program reporting and document client care, evaluations, and follow-up in a clear, timely, and legally compliant manner

• Identify and report potential or actual unsafe situations to the appropriate professionals

• Advocate for client access to needed programs, promote healthy public policy, and support coordinated responses to health issues

Education and Research

• Provide health education that supports individuals, families, and groups in building knowledge, life skills, and healthy practices

• Contribute to home care practice through mentoring, role modelling, professional development, and self-reflective practice

• Gather and share relevant health information from clients, families, community leadership, and health practitioners in a clear and accessible way

Organizational Responsibilities

• Plan and deliver preventative health gatherings, workshops, information sessions, staff development activities, and related education as required

• Work collaboratively with home care workers and health professionals to review caseload priorities, make referrals, and support Home Care Program planning and evaluation

• Maintain confidentiality, follow Health Centre standards, policies, and procedures, and bring concerns about professional practice standards to leadership as appropriate

What You Bring:

• 2-3 years of experience in a similar position

• Experience working in and with an Indigenous community is preferred

• A Certificate or Diploma in Health Sciences

• Experience in community health is an asset

• Current CPR Certificate

• A valid driver’s license

• Experience working with multi-disciplinary and inter-disciplinary teams

Other:

• Criminal Record Check and Vulnerable Sector Check with a satisfactory result

• Ability to work flexible and irregular hours

• Maintain and update relevant immunizations

• Participate in professional development and maintain position credentials

• Hours of work: 35 hours per week

• Schedule: Monday to Thursday from 8:30 am-5:00 pm, and Friday from 8:30 am-1:00 pm
